Before You ComeBefore you come for an appointment, it’s a good idea to do the following things, if possible: 1. For your first appointment with us, get a copy of any previous medical records for your pet from any other veterinary hospitals that have seen your pet – or call them to have those records faxed to us. If you can fill out the appropriate forms below ahead of time, that will save some time as well. Avian – New Patient Form – 2009 2. Make notes concerning the caging, diet, and general conditions of your pet – we will ask you to be detailed on our questionnaire! 3. Make sure you know how to get to our hospital. 4. Contact us to find out if you should bring a fresh stool or dropping sample from your pet. 5. Make sure you have an appointment – walk-ins/work-ins will usually have a significant wait time, and a daytime emergency fee may be added to the visit if the appointment schedule is already full. 6. Please realize that we may have to consider laboratory testing or other diagnostic means to determine if your pet is sick. 7. Please understand that we are a veterinary hospital that performs some grooming services for our clients – therefore, all of our patients that need a nail trim or a wing feather trim must have a physical exam done within 6 months of the grooming service.
